Medication Safety is a vital part of every patient's visit to the hospital. This means, we want to make sure you receive the medication prescribed to you at the right time in the correct manner.
In order to promote Medication Safety at Medical West, we follow a Medication Reconciliation process, which requires nurses, doctors, and pharmacists to check and recheck each patient's medications to prevent any errors. It is extremely important for your health care providers to have a complete and accurate list of your medications so that they can provide you with the best, most personalized care. We believe active involvement by you, the patient, can greatly increase the success of this program. We have developed the Medical West Med Card to assist you in keeping track of all of your medications. We encourage you to carry this card with you to every visit to the doctor, stay at the hospital, or any other interaction with a health care provider. The Medical West Med Card should be initially filled out by you, your primary care doctor, or your pharmacist and should be updated each time medicines are added or omitted.
The Medical West Med Card will not only help you keep your medications organized, but will also assist you in discussing your medical history with your doctor, and provide your health care providers with more complete and accurate information. Such communication is crucial to providing you with a high level of care and to avoid drug interactions.
