CPE Application

In order to apply for CPE you must complete the standard ACPE application form.

We have two methods for you to acquire this application:

First, you can download the application form in Adobe Acrobat or MS Word format by clicking on one of the links below.

Second, you can call, write, or email to receive a CPE application via mail. After completeing the application, simply mail it with the current application fee.

Once you have completed the application:
