With provision of coordinated care,
Family Clinic strives to ensure that medical and supportive services work together. The services provided by our program include multidisciplinary medical and psychosocial care of all HIV-infected patients. Population-specific providers include pediatric infectious disease specialists, adolescent care providers and adult care providers who focuses on the individual needs of each population.
In addition, other
Clinical services available to our clients include case management, pediatric/adolescent mental health, adult mental health, counseling and referrals, peer and family advocacy, family planning, pharmacy, and nutrition.
The Family Clinic enhances the resources available to our clients by working and collaborating with a coordinated network of providers across the state and within each client's community.
Supportive services include transportation, support groups, consumer and family advocacy, counseling of HIV-infected pregnant women about infection risk to their children, consumer and parent comprehensive education about HIV transmission and its complications, and coordination with other agencies to provide necessary services such as housing, substance abuse services, credit counseling resources, and other basic needs for our clients.
When making your first appointment you will need to provide your name, address, phone number, social security number, insurance information, and a contact name and phone number in case of any emergency. The first visit will include an intake assessment with the social worker. At that time an appointment to discuss your health care with a physian will be scheduled. You can expect this visit to take 2-3 hours.
