Driving Evaluations
To schedule a driving evaluation for your patient you will need to fax your patient's demographics, a copy of the prescription for the evaluation and a current H&P or DC summary that summarizes your patient's condition and ability to complete the evaluation. If your patient has been treating here at UAB include their UAB medical record number with your request and you will not have to include the medical records.

Hearing Evaluations
Our hearing evaluations are done by Audiologist. Once the evaluation is complete we will meet with the patient and review the results and discuss the best options for hearing aide devices. To refer a patient for our services you will need to fax a pre-authorization including all contact and billing info, your patients demographics and contact information and your patient will be contacted with an appointment. If we do not obtain the preauthorization your patient will be billed for the services. Please include a request for a copy of the report with your contact information and it can be mailed or faxed to you.

New Locomotor Training Program
UAB Spain Rehabilitation Center's new Locomotor Training Program on the Therastride body weight supported treadmill system provides evidence-based retraining of the nervous system to improve walking in neurological dysfunctions including stroke, incomplete SCI, Parkinson's and others.
The new system is located in room 700 of the CPM building.
