To set up an appointment with a physician in the UAB Health System, please follow these simple instructions:
1. Contact International Patient Services:

2. Obtain the files of the clinical history from the attending physician. These may be in any language; however, if possible, and to accelerate the process, we prefer that everything be sent in English.
3. Include the most recent results of special examinations,
laboratory tests and diagnostic images. If some of these
examinations have not yet been made, these can be
carried out at UAB, as necessary. Please be sure that all
the examinations have the name of the patient and the date
on which each one was done.
4. For patients with heart, cancer, or neurological problems:
a. Patients with heart problems
- Electrocardiogram
- Ecocardiograph report
- Catheterization (the most recent film and corresponding
information)
- Report from Holter monitor (for patients with arrhythmias)
b. Patients with cancer or hematological problems
- Scans and/or magnetic resonance imaging
- Tumor markers
- Bone gamography
- Analysis of HLA (for patients referred for evaluation
of medula osea)
c. Patients with neurological problems
- Scans and/or magnetic resonance imaging
- Electroencephalogram
- Catheterization (for patients with cerebrovascular
problems) If already performed, include the film and
corresponding report.
TO AVOID REPEATING EXAMINATIONS IT IS
RECOMMENDED THAT THESE BE DONE IN
THE UAB HOSPITAL
